I am a big fan of this site. We use it extensively at United Way. Formsite allows you to build fairly sophisticated online forms. It is backed up by a database so that all the information captured can be download into a spreadsheet. The form building tool is intuitive and you can have a form up in no time. You can even play around with a free version before you purchase the services (annual subscription). This is the kind of tool you could sign up for with a couple other agencies to share the cost.  Click here for samples.

This site has many tools you use. Membership is free. They have guestbooks, mini surveys, email lists and ezines, a survey tool, a way to password protect a page or pages, and much more. I have used their services on occasions and they are quite reliable.
